NOTICE TO PATIENTS 

Salem Family Medicine will be closing effective April 26, 2024. We cannot express with words how grateful we are to our patients for entrusting us with their care over the years. Moving forward, we recommend our patients reach out to LewisGale Physicians, Carilion Clinic, or other primary care practices in the Salem area for their continuation of care.   • Medicaid policies as a primary insurance. This includes Medicaid policies that are labeled as Anthem Health Keepers Plus, FAMIS, Commonwealth Coordinated Care (Medicare/Medicaid Dual Integration), and CareMore

  • VA Premier

Please Note

  • Salem Family Medicine accepts patients 5 years and older who have completed all primary immunizations according to the latest recommendations from the AAP, AAFP, CDC and the Health Boards of the Commonwealth of Virginia.

  • We do not administer Department of Transportation Physicals.
